Please call our office to schedule an appointment, or send us an email with your full name and phone number and we will be in touch within 24 hours. In the mean time, feel free to click the New Patient button here in order to fill out all necessary personal information and forms for your initial visit. You will be prompted to our patient portal where all information collected is securely stored and will remain as part of your confidential health record.

If you have not been back to Northern Comfort Wellness in the past three years, please fill out new patient forms to provide us any necessary history we will need to provide you the best health care.
